Undoubtedly, many of your patients that need Medicaid have income above the Medicaid limit. But that should not stop them from getting Medicaid. Setting up a pooled income trust is great way for someone who needs home care to access Medicaid benefits without having to “spend-down” their excess income. If your patient has Medicare (or managed Medicare – also known as Medicare Advantage) and needs home care, we can assist with obtaining care covered by insurance. We are contracted with a number of Certified Home Health Agencies (CHHAs) and are often able to make arrangements even for difficult cases. The free Medicaid assistance we offer can often be a key tool in arranging a discharge, even for patients who at the time of discharge only have Medicare.
Medicare services tend to be limited both in terms of the amount of services provided and their duration. However, they can be helpful for someone who needs help adjusting at home after a hospitalization or stay in a rehab. Additionally, they can help bridge the waiting period for someone who needs long-term home care and is working on a Medicaid application.
Home Care Arrangements for Patients with Medicaid
The process for obtaining home care services for patients who have Medicaid or are dual-eligible (meaning they have both Medicare and Medicaid) has become more difficult and cluttered with red-tape than ever. We are contracted with a number of Certified Home Health Agencies (CHHAs) and are often able to make arrangements even for difficult cases. Additionally, we are contracted with most Managed Long-Term Care (MLTC) plans, an often overlooked option that can sometimes be a true lifesaver when arranging a patient’s discharge.
Medicaid Pending Home Care Arrangements
If your patient needs significant home care immediately upon her return home, you may want see if she can receive services Medicaid pending. This means that an agency would provide your patient with Medicaid level home care services while her application is being processed. Eligibility for Medicaid pending services is determined on a case-by-case basis, and not everyone who qualifies for Medicaid will be approved for Medicaid pending. That being said, in many cases it can be a real lifesaver.
You may have heard that Medicaid pending is no longer available. This is inaccurate. While it has gotten harder to get approval, Medicaid pending is still very much available.
